Forma

The med spa EMR built around how you run.

Booking, charting, photos, consents, inventory and checkout on one record, set up around your services, your protocols and your team. Missing something? If it belongs in Forma, we build it in.

Shown with an example practice, Juniper Aesthetics. Names and data are made up.

Four tools, one system

Today, Ava lives in four places.

Her booking is in one app, her texts in another, her consent in a folder and her membership balance somewhere else. In Forma she's one record, and everyone in the practice sees the same one.

  • 1

    Storefront & loyalty

Your services and treatment packages on a page with your name on it, and memberships that track her credit balance.

  • 2

    Scheduling & payments

Providers, rooms and the front desk on one calendar, and checkout at the desk.

  • 3

    Messages

Portal chat and her message history, in the same place as her record. Text and email sending is in progress.

  • 4

    Photos & consents

Before and after photos and signed consents, filed on the visit they belong to.

A visit, start to finish

Follow one patient through one system.

Ava books on Sunday night, signs her forms from her phone, is treated on Tuesday and checks out at the desk. Every step lands on the same record.

    She books from your storefront.

She fills in intake and signs consent from the client portal on her phone, or on the iPad in the room, and the signed copy files itself on her record.

Charting does the paperwork

Chart the point. The stock and the bill are done.

When a provider saves a point on the injection map, Forma takes the units out of the right lot and adds the line to the visit's draft invoice, in one step. No end-of-day tally, and nothing to type twice at checkout.

  • The right lot, every time

Units come out of the lot closest to its expiry date. Expired lots are never deducted from.

  • Billed as it's charted

Each point adds its line to the visit's draft invoice, priced from the product. Checkout starts with the treatment already on it.

  • Fix a point, both follow

Edit a point and the stock and the invoice line move with it. Delete one and the units go back on the shelf.

Stock moves when the practice has Deduct automatically on (Settings → Inventory). With it off, charting still records what was used and still bills for it.

How we work

Built around how you run.

No two practices run the same way. We sit down with your team, map how your practice works and set Forma up to match.

  1. 01

    Meet

A working session with your team. We learn how your day actually runs.

  2. 02

    Map

Your services, charts, forms, memberships and vendors, written down together.

  3. 03

    Set up

Forma configured to match, using the settings you see here.

  4. 04

    Move

We move your clients, services and appointments for you, carefully, after the BAA is signed.

  5. 05

    Keep tailoring

Regular check-ins after you go live. Requests that fit become settings every practice can use.

  • A BAA comes first

We sign a BAA with your practice before any patient data moves.

  • Missing something? We build it in

It goes into Forma as a setting every practice can use, not custom code for one practice. We'll tell you plainly what fits.

  • Where we say no

Anything that weakens consent, records or privacy. Those stay as they are, whoever asks.

Runs the practice with you

Rules where rules work. AI where it saves real work. People decide.

Forma already takes routine work off your team with rules. AI comes next, only where it saves real time, and a person always makes the call.

Rules

The routine, handled

Live

Predictable work runs on rules you can read. No AI needed.

  • Charted units leave the right lot
  • Soonest-expiring lot first; expired lots never used
  • Lots expiring within 30 days flagged
  • Consent expiry tracked per form, 12 months by default
  • Best waitlist match flagged when a visit cancels
  • Auto-reorder: a draft PO at the reorder pointOn the roadmap
  • Treatment-interval rebookingOn the roadmap

Assist

AI drafts, people decide

Coming

AI takes the first pass at the writing. A person reviews, edits and signs.

  • Ambient charting scribe: drafts the note, the provider signsOn the roadmap
  • Intake review that flags contraindications for the providerOn the roadmap
  • Campaign writer in your practice's voiceOn the roadmap

In Assist, nothing goes to a patient until a person on your team sends it.

AI front desk

Coming

Makes your best person faster. Covers the phones when they're busy.

Assist
Works beside your receptionist: the caller's record opens before hello, replies are drafted for them to edit and send, and clients who are due are suggested.
Answer
Picks up calls and texts after hours and when every line is busy. It says it's an AI assistant, anything clinical goes to a person, and it only does what you've switched on.

Our AI principles

  1. 1Rules first. If a rule can do it reliably, it stays a rule.
  2. 2AI drafts, people decide. Nothing clinical is sent or signed automatically.
  3. 3Never a clinician. Our AI receptionist will always say it's an AI, and hands anything clinical to a person.
  4. 4Patient data under a BAA. No training on your practice's data.
  5. 5It explains itself. Every suggestion shows why it was made.

The platform

What's in Forma today, and what's next.

Seven parts of the practice, one record underneath. Each capability says where it stands, so you know exactly what you get on day one.

Security

How patient data is protected, stated plainly.

Forma is hosted on AWS. We sign a BAA with your practice before any patient data moves. And these controls are in the product today:

  • Role-based access

Owner, admin, provider, staff and read-only roles. Providers see only their own clients.

  • Automatic sign-out

After 15 minutes idle for staff, and 30 minutes for clients in the portal.

  • Each practice kept separate

Every practice's data is isolated, and automated tests check that it stays that way.

  • Record access logged

Each time someone opens patient records, it's logged.

  • Nothing hard-deleted

Archived client records are kept, never hard-deleted.
